APPROVED: (Any of the following) Inspector found no violations.
Three or fewer critical violations.
Inspector found noncritical violations.
(Reinspection is not required):
UPGRADE: Restaurant passed second inspection within 10 days after an unsatisfactory inspection.
UNSATISFACTORY: Four or more critical violations, a repeat of a critical violation or the fourth repeat of a noncritical violation.
PERMIT SUSPENSION/CLOSED: Permit is suspended if all critical violations have not been corrected within 10 days, or when an imminent health hazard is present. Restaurant is closed until suspension is lifted.

FOLLOW-UP: Cream gravy on steam table found at 130 degrees. Pinto beans at 132 degrees. Discard. Red chile at 143 degrees; green chile at 157 degrees. Make sure food is reheated to at least 165 degrees and then hold at 140 degrees or higher. Improper hand-washing observed. Inspector saw cook rinse gloved hands after handling eggs. Gloves must be removed before washing hands. Fresh gloves must be used before handling ready-to-eat foods. Sanitizer solution tested at 0 ppm quaternary ammonium and 0 ppm chlorine in both wiping cloth buckets in the wait station area. Call for repair of dispenser. Employees must mix solution by hand and test to verify concentration until the chlorine is delivered. Note: Restaurant must use chlorine sanitizer until further notice. Chlorine has been ordered and will be delivered soon. Wiping cloths and dirty knives and utensils stored in a bucket of sanitizer solution. Slicer has been cleaned, but food debris remains on the machine. Plumbing leak near dishwasher. Ice buildup on pipes in freezer. Plumbing violations noted on previous inspection have been repaired but additional problems need to be fixed. Food equipment stored in employee restroom. Wall is damaged near the dishwasher. Ceiling tiles are damaged. Towels found stored in the employee restroom. Garbage area is dirty and greasy. The Dumpster is shared by many businesses in the shopping center. Area must be cleaned. Grease bin lid is open. Lid must be kept closed. Area has been cleaned since the previous violation but cleaning schedule must be increased to prevent attracting pest. Call for follow-up as soon as violations are corrected.
