Dragonhorn Tavern, 2906 Juan Tabo NE

Debris and buildup were found inside the soda gun dispenser holder in the bar area. The bar hand-washing sink was non-operational, with a “do not use” sign and a drain pipe that was too short. There was an inadequate air gap for the ice machine drain pipe located at the floor drain. The establishment did not have a certified food protection manager on staff. Employees did not have valid food handler cards or approved training records. The facility lacked a commercial-grade ventilation hood system for the fryers and flat-top grill. The establishment was found to be operating with an expired permit. No hot water was available at the hand-washing sinks in the restrooms. No written notification regarding major food allergens was provided for unpackaged food items. The menu lacked a proper disclosure and reminder regarding the risks of consuming raw or undercooked animal products.

Frost Gelato, 2201 Q NE

Gelato mix in the reach-in refrigerator lacked the required date marking indicators. The establishment did not have a certified food protection manager on staff. Food employees did not have valid food handler cards or approved training records. Gelato and frost bites inside the cold-holding units were not covered to protect them from contamination. The doors of the facility had gaps that were not tight-fitting to minimize the entry of insects and rodents. Substantial holes were observed in the structure of the facility near the back door.

Goodfellas Pizza, 5844 Osuna NE

Dried food and soil debris buildup were found on the can opener and mixer. A spray bottle of window cleaner was not properly labeled with its common name. A floor drain was missing its protective dome cover. The food establishment did not have a certified food protection manager on staff. Food employees did not have a valid food handler card or approved training. Debris buildup was observed on the interior intake and exhaust air ducts in the kitchen. A large white food bin containing flour was not identified with a common name label. The men’s employee bathroom in the kitchen area was not equipped with a tight-fitting, self-closing door. Light was visible through the edges of the back dock area garage door. Soil debris buildup was found on the faucet and sink fixtures in the kitchen. Light bulbs in the kitchen and the walk-in cooler were missing protective covers or shielding. A light bulb burned out inside the walk-in cooler. A food employee was actively working with open food items without proper hair restraints. The facility was found to be operating with an expired permit. Buildup and debris were found on the floors and walls under the three-compartment sink and walk-in cooler. No written notification of major food allergens was provided for unpackaged food served to consumers. Multiple food containers had portion cups stored inside them, directly touching the food product. Dried food and soil debris buildup were found on the underside of kitchen shelving. Floor and wall juncture covering had come off the wall in the warewashing area. The establishment did not have temperature logs available for time/temperature control for safety foods. Calibration logs for temperature-measuring devices were not available.

Pollito con Papas, 6105 Gibson SE 

Chlorine and quaternary ammonium sanitizing solutions failed to meet required concentration and temperature parameters. Wiping cloths were being used improperly. Multiple ready-to-eat items in the walk-in refrigerator were held without any date-marking indicators. Waste bins were in poor repair, with no lids and substantial debris buildup in the surrounding area. The person in charge was unable to demonstrate knowledge regarding holding temperatures, foodborne illnesses or proper sanitizer concentrations. Written procedures were not available for multiple food items stored at room temperature, using time as a public health control. The establishment did not have a certified food protection manager on staff. Employees did not have valid food handler cards or approved training records. Heating and air conditioning vents on the main kitchen line had substantial debris buildup. Salt, oils and other ingredients were stored in working containers without labels identifying the common name of the food. Cardboard and foil were absorbent, nonsmooth surfaces inside the walk-in refrigerator and on the stove top. The toilet room was not equipped with a tight-fitting, self-closing door. Outer openings were not tight-fitting and allowed light to be seen at the edges. Multiple holes and gaps of various sizes were observed in the walls and structure of the kitchen and prep areas. No testing kits were provided to measure the concentration of the chemical sanitizing solutions. The facility did not provide a designated area for employees to eat, drink or use tobacco. A light bulb was missing from underneath the hood suppression system. Employees were observed wearing bracelets while preparing raw meats and entrees. The facility was operating with an expired permit. Employees failed to wash their hands when switching job duties or before donning new gloves after preparing raw meat. Multiple soiled mops were stored improperly in a mop bucket instead of being allowed to air dry. Buildup and debris were found underneath cold-holding units, the three-compartment sink and shelving. A clean cutting board was stored improperly in a window seal. Temperature logs for stored, received and thawed foods were not available. The establishment lacked calibration logs for its temperature-measuring devices.

Quick Track, 2625 Wyoming NE (Class C) 

Cutting surfaces and pizza-baking pans were found with a buildup of soil residues or encrusted grease. Cooking equipment was not being cleaned and sanitized between uses. An unmarked plastic spray bottle was found with an unknown material. Sliced jalapenos and pepperoni were held in the prep top cooler without date-marking indicators. No person in charge was designated at the time of the inspection. The floor drain of the three-compartment sink was draining slowly and causing a sewage backup. The faucet of the three-compartment sink was leaking. The establishment did not have a certified food protection manager on staff. Food employees did not have valid food handler cards or approved training records. No testing kit was available to measure the concentration of the chemical sanitizing solution. No handwashing cleanser was available at the restroom handwashing sink. The handwash station near the pizza oven was being used to store cleaning utensils and discard drinks.
