The ice machine contained black mold-like buildup, while the can opener and deli slicer had food debris accumulation. Poisonous chemicals like degreaser and glass cleaner were stored directly next to ready-made tea at the beverage station. A plastic spray bottle containing degreaser was not properly labeled with its common name. The floor drain near the beverage station was in disrepair, with excessive food debris and trash buildup. Inadequate air gaps were observed at the floor drains near the warewash unit and under the beverage station. The hood system was dripping grease and condensation onto the fryer equipment below. Lighting in the hood vent area was inadequate due to a burned-out bulb. Extreme grease and food debris buildup were found on stainless steel and plastic surfaces throughout the facility.

Fun Noodle Bar, 5317 Menaul NE
The facility demonstrated inadequate procedures to effectively control pests. Over 10 live cockroaches were observed in various locations, including the janitor sink area, near the ice machine and on ceilings and walls around prep coolers. An inadequate air gap was observed at the drain located under the prep table near the noodle-making station.

Sonic Drive-In, 2119 Vista Oeste NW
The ice machine interior contained black soil residue, and the tater tot hot-holding unit had substantial soil buildup. Ready-to-eat foods like red chili and hot dogs in the steam table were found with inappropriate or incorrect date markings. The scrapper sprayer at the three-compartment sink was leaking, and the water valves failed to shut off completely. The hood system was inadequately collecting grease, causing significant accumulation and dripping. Debris and buildup were observed on floors throughout the cook, prep and storage areas, as well as under equipment. Floor drains in the food prep and cooking areas contained excessive black soil buildup and debris. Corner walls near the drive-thru were found to be not smooth or easily cleanable.

Takumi Restaurant, 2100 Louisiana NE
Wiping cloths used for cleaning were improperly stored on a bus cart and above prep tables instead of in a sanitizer solution. Ready-to-eat chopped vegetables in the standing cold holding unit lacked the required date markings. The faucet at the three-compartment sink was leaking and unable to shut off completely. The establishment did not have a certified food protection manager on staff. Substantial food debris was observed on the kitchen vents, surrounding ceiling tiles and light covers. Debris buildup was found under cold holding units, prep tables, sinks and inside refrigerators and the dish machine. The facility did not maintain temperature logs for TCS foods like raw meats and dairy. Calibration logs for temperature-measuring devices were not available.
