Don Sushi, 205 Silver SW

Debris buildup and dead cockroaches were found inside the ice machine and its filter area. Twenty dead cockroaches were found accumulating in traps in the dining area, dry goods area and inside the ice machine. Ready-to-eat TCS foods in the sushi cooler lacked required date-marking indicators. The person in charge was unable to demonstrate knowledge regarding foodborne disease prevention and Food Code requirements. An inadequate air gap was observed under the sinks located in the bar area. The establishment did not have a certified food protection manager on staff. Food employees did not possess valid food handler cards or proof of approved food safety training. Reused containers filled with herbs and flour in the dry storage area lacked proper identifying labels. A bucket of salsa was held at an unsafe temperature of 47 degrees in the bar area’s walk-in cooler. The facility was observed preparing sushi rice without the required HACCP plan. Ambient air temperature-measuring devices were missing from the sushi reach-in cooler. A food employee in the sushi area was observed preparing food without the required beard restraint. The handwashing sink in the bar area lacked sufficient hot water, reaching only 77 degrees instead of the required 85 degrees. The facility’s in-house parasite destruction documentation for fish was found to be invalid or incomplete. Debris and dead cockroaches were found on the floors in the dining areas, by the bathrooms and on electrical cords above the sushi area. Debris buildup was observed on the exterior roofs and vent covers of the walk-in coolers and water heater. Holes in the walls and missing ceiling tiles were observed in the dry goods storage, water heater closet and kitchen areas. The facility did not maintain temperature logs for cold and hot holding foods, equipment or sushi rice. Calibration logs for temperature-measuring devices were not available.

Filiberto’s, 1450 Eubank NE

Black, yellow and brown organic buildup was found inside the ice machine, and dried food debris was observed on the food dicer blade. Soiled wiping cloths were improperly stored on prep tables and food containers instead of in a sanitizer solution. Ready-to-eat TCS foods in the prep, upright and walk-in coolers lacked required date marking indicators. The establishment did not have a certified food protection manager on staff. Debris buildup was observed on the interior intake and exhaust air ducts in the kitchen. Working containers of sugar, salt and pepper in the dry storage area lacked proper identifying labels. The women’s restroom lacked the required covered waste receptacle for sanitary napkins. The back door in the kitchen was not tight-fitting, allowing light to be visible at the edges. Foods, including lettuce, tomatoes and sour cream, were stored improperly on the floor of the walk-in cooler. The facility did not provide lockers or a suitable designated area for employees to store personal belongings. Excessive soil and debris buildup was observed on the soap dispenser at the main cook line handwashing sink. The men’s restroom handwashing sink lacked a required sign reminding employees to wash their hands. The facility was observed operating with an expired permit. Buildup and debris were observed on the walls and ceiling in the kitchen area. The facility did not provide written notification of major food allergens for unpackaged food sold or served to consumers. Multiple ceiling tiles throughout the facility were water-damaged and warped. The facility did not maintain temperature logs for stored TCS foods.

Rio Grande Presbyterian Church, 600 Coors NW

Heavy amounts of rodent droppings were observed throughout the facility, including in dry storage and walk-in coolers, with no pest control methods in place. Waste bins at the outdoor dumpster were found without tight-fitting lids or doors. A floor drain grate was found in disrepair, failing to fully cover the drain trough and allowing a potential entry point for pests. Holes measuring 3-5 inches were observed on the bottom side of the wall and foundation on the rear side of the facility. A significant amount of nonfunctional kitchen equipment and cardboard were stored inside the pantry and on the building exterior. Buildup and debris were found on floors throughout the facility, including inside the walk-in coolers and underneath shelving. Holes and large openings were observed in the walls and base coving in the hallway in front of the walk-in coolers.

Subway, 1100 Central SE

The quaternary ammonium sanitizer used for warewashing did not meet required parameters, prompting the person in charge to remake a fresh batch. Twenty to 30 dead cockroaches were found accumulating in traps near the beverage station wall and cabinets. The person in charge lacked the necessary food safety knowledge to identify and prevent signs of pest infestation. The PIC was unable to provide the required training records or certifications for the food employees. Foods on the make line, including guacamole, green chile, chicken cubes and ham, were held at unsafe temperatures above 41 degrees. Ambient food temperature-measuring devices were missing or not readily accessible inside the make line’s cold-holding units. Employee personal care items were improperly stored directly on or next to kitchen equipment. The mop sink was backed up and not free-flowing, preventing proper wastewater disposal. Nonfood-contact surfaces throughout the facility, including the microwave, shelving, prep tables, beverage machines, carts, floors, walls and equipment exteriors, were heavily soiled with debris and food residue.
