Result: Immediate Closure

Observed interior of the refrigerator with caked on soil build up on the bottom and holding racks. Observed refrigerator unit not holding time controlled for food safety foods below 41 degrees. Raw beef holding 56.05 degrees. Potato salad holding 59 degrees. Queso holding 52.05 degrees. Observed no hot water or cold water available for hand washing and dish washing.

Observed facility has no means of providing adequate cold holding for TCS foods requiring that holding facility’s only refrigeration unit was not functioning. Also no running water to hand wash and to clean and sanitize at the three-compartment sink. These are imminent health hazards. Facility is closed and will contact environmental health once everything is brought to compliance.

Observed hand wash sink not operating to no water in the mobile unit.

Denny’s, 5201 San Antonio NE (July 5)

Result: Immediate Closure; passed follow up inspection.

Facility has evidence of ongoing roach infestation resulting in immediate closure of facility. Person in charge failed to ensure that employees follow necessary procedures to properly prevent contamination and protect consumers and staff from an imminent health hazard.

Observed reach in cooler in the kitchen area and saute station with significant debris such as liquid pooled eggs on the surfaces.

Observed small gaps along the bottom of the back door leading to the outside by the dish area. Observed live roaches within the kitchen at the egg station by floor drain, and behind egg station trash can and underneath the prep cooler, and table where plates are stored. Pest control treated the facility on June 26 and was to return due to live activity found on July 2, however CHPD did not see record of their return.

Observed significant amounts of dead roaches throughout facility including in areas such as mop sink, chemical storage, dish machine, pantry, server line and throughout kitchen under equipment and inside equipment cubbies.

Observed fixed equipment in the kitchen with plumbing lines, gas lines and openings behind with no ability to clean the area. Observed significant food, debris and grease build up on the sides, on the casters, underneath and on top of equipment such as flat top, fryer, reach in coolers, saute station and dish machine area. Observed debris build up in service area cabinets and storage areas.

Observed significant debris build up inside the floor drains throughout kitchen and dish area. Observed missing floor drain covers in the kitchen. Observed significant heavy amounts of food debris and grease build up on the floors and walls throughout facility, under and behind hand washing stations, equipment under and in openings behind the equipment such as reach in coolers, fryers, dish machine.

Observed holes in walls and ceiling throughout facility in areas such as at plumbing fixtures by mop sink, electrical outlet with no plate cover at kitchen pass through, small holes in wall across from oven, by omelet station reach in cooler, by beverage lines into ceiling, at the wall and shelving next to the back door in the server station, behind the cooking equipment at electrical lines, and seams of drywall, and metal corner molding coming off of walls exposing gaps into the wall at kitchen pass through and the wall’s end by the utensil rack/combi-oven. Observed grout having eroded away or missing in the dish area, and kitchen with pooling water, grease and food debris inside.

Observed debris build up on ventilation system and the surrounding area in the kitchen and service. Observed significant grease build up in hood system.

Roach infestation within the kitchen area, with live roaches present at egg station grill and prep cooler. Person in charge was issued a notice of closure and suspension.
