Result: Immediate closure; passed follow-up inspection

Observed uncovered containers of cream gravy, biscuits, pancake mix. Observed debris build up inside ice cream reach in freezer. Observed debris build up inside reach in coolers and freezers as well as in walk in freezers. Observed bacon, buttermilk, boxes of eggs, cheese, squash, onions, lettuce, jalapenos, beaten eggs all out of temperature. Observed egg station cold holding table and cooler (60 degrees) temping above the allowed 41 degrees as well as walk in cooler temping (55 degrees) above 41 degrees.

Observed food and non-food contact surfaces improperly designed, constructed, installed, used or not kept in good repair. Observed debris build up on the inside and outside of the deep fryers, ovens, prep table and on reach in coolers and freezers shelving. Observed walk in cooler with holes and tear in roof areas, seal/seam. Observed debris build on windows of doors to access front area to the kitchen area and on top of the ice machine and on reach in coolers and freezers doors and handles. Observed standing water under sink located in kitchen area near oven clogged and a leak under sink on the p-trap. Observed standing water underneath hand washing sink in the back area next to the dish washing area.

Observed debris build up on walls and floors throughout the facility. Observed hole in ceilings in the kitchen area line where food is being prepared where AC ducting lines have been installed.

Observed debris build up on hood filters in the kitchen area. Instructed person in charge to clean hood filters to come into compliance and send a picture of the cleaning by July 30. Deadline was reached and no pictures or cleaning had taken place.

Observed missing vent covers in dish washing area above dishwasher and above dish washing station.

Observed cook pick up wrapper of food off floor and proceed to start touching cold holding tomatoes. Observed no soap at hand washing sink.

Church’s Chicken, 2937 San Mateo NE (Aug. 3)

Result: Immediate closure

Repeat violation. Observed harborage, evidence or presence of rodents, insects, and animals in facility.

Observed pest activity, cockroaches, in back room where single service items are stored in same area cockroaches seen behind reach in freezer, on walls in same area as well as cockroaches seen dead and alive in the kitchen’s cooking area and in the men’s bathroom and in the guards on the lighting in the dining room area dead cockroaches were visibly seen. Instructed person in charge that there is a heavy accumulation of pests (cockroaches) inside the facility and that this issue needs to be addressed and abated for the safety of the customers/community.

Observed debris build up on hot holding area where chicken and other items are being stored. Observed debris build up on top of shelving in the kitchen’s prep area as well as on the outside of the reach in coolers. Observed toilets inside the women’s bathroom and men’s bathroom with debris build up on and inside the toilet. Observed debris build up on floors in throughout the kitchen and dining area. Observed missing floor tiles in kitchen and dish washing area. Observed holes in ceiling area by hot holding case as well as holes in ceiling above hot water heater.

Immediate closure due to pest activity (cockroaches) inside the facility. Inspector visibly saw alive and dead roaches inside the facility’s kitchens, dining area, back room, where single use items are stored and in the men’s bathroom.

Hooters, 4601 San Mateo NE (Aug. 4)

Result: Immediate closure

Observed ice well drainage tube disconnected, draining onto the floor with pooling of water.

At the time of inspection, roach infestation within the kitchen, along the kitchen bar, behind cooking equipment, such as flat top, and behind the dish machine in the prep area. Person in charge was issued a notice of closure and suspension and has five business days (Aug. 11) to cease roach infestation and to remedy the infestation.

Facility had two priority violations and evidence of ongoing roach infestation, resulting in the immediate closure of facility. Person in charge failed to ensure that employees follow necessary procedures to properly prevent contamination and protect consumers and staff from an imminent health hazard.

Observed raw shell eggs stored below multiple containers of raw chicken. Observed gaps along the bottom of front and side doors leading to the outside. Observed exterior wall to west of main entrance at the garden bed with overhang where the bottom had fallen off exposing interior, broken windows and window frames in poor repair with entrance points. Observed dead roach in this area. Observed significant debris and grease build up on, inside and on gas lines throughout kitchen equipment, including flat tops, fryers, grill, and ware washing machines.

Observed three-compartment sink, ice well, hand washing sink with plumbing lines in the floor drains below the flood line with no air gap allowing for the possibility backflow of sewage into the three-compartment sink. Observed significant debris build up underneath kitchen cooking line equipment.

Observed throughout the facility heavily prevalent gaps and holes in areas such as at the counter service area: at the wall juncture to the bar top, gaps around the FRP wall material junctures, door frames, electrical outlets with and without plugs attached, around beverage lines, corner junctures, at areas where material had been cut out. Around door frames from kitchen to prep area, kitchen to storage area, prep area to the dry storage. At the dish area gaps around plumbing and electrical lines into the wall, and into the ceiling tile. Broken ceiling and missing ceiling tile above ice machine. Gaps around gas lines into the wall behind cooking equipment. Observed coving tile throughout kitchen, prep area and storage rooms coming unsealed to the walls with gaps present. Observed flooring throughout kitchen, prep area, dish area, and storage rooms with cracked, missing tiles and with grout having missing/eroded away with pooling water and debris.

Evidence was observed to substantiate the citizen complaint of roaches in the facility.

Kid’s Castle, 12840 Lomas NE (Aug. 3)

Result: Immediate closure

Observed raw shell eggs stored in reach in cooler temping at 60 degrees. Observed bags of cubed cheese stored in reach in cooler temping at 64 degrees. Observed reach in cooler ambient temperature temping at 61 degrees. Observed digital temperature gauge on cooler display reading 73 degrees. Instructed person in charge to dispose of all food items in the reach in cooler, which included: five bags of cubed cheese (five pound bag); four flats of raw shell eggs; six bottles of almond milk; 16 butter (one pound); 96 yogurts (four ounce containers); three containers of plain yogurt (two pound); three containers of ricotta cheese; one carton of cream cheese; two jars of ranch dressing (five pound); two bags of mozzarella cheese (five pound); four half gallons of lactose free milk; 168 string cheese (one ounce); two bags of baby carrots.

Observed live cockroaches of various life stages in the following locations in the facility: numerous live cockroaches inside the drawers in kitchen area; on the walls of the cabinet areas; on the floor underneath the shelving adjacent to cabinets near hand washing sink. Dead cockroaches were observed in the following locations: in the cabinet underneath the handwashing sink; inside utensil drawers; inside cabinets used to store clean cloths and towels; on the floor in kitchen area.

Observed uncovered cups of graham crackers stored on the shelf of the dry storage area. Observed three holes in the wall near hand washing sink.

Facility’s kitchen is closed effective Aug. 3 due to the imminent health hazard of lack of refrigeration.

Kabob House, 8104 Wyoming NE (Aug. 2)

Result: Immediate closure; passed follow-up inspection

No person in charge was present. Owner arrived after employee called when CHPD arrived. Owner became aggressive and made threat during inspection towards inspector. CHPD left, but came back with assistance of pest control services in order to work with facility.

Observed in reach in cooler raw chicken skewers stored above squeeze bottles of multiple sauces, container of cooked rice, containers of pickled cabbage, raw beef skewers and chicken skewers above diced onions, and currants, and in second reach in cooler containers of raw chicken stored above items such as cut tomatoes, sauces and herbs.

Observed heavy rodent droppings in the kitchen where onions and other food items were stored, behind stove and grill, at the ware washing equipment, in the server area and beverage station. Pest control services came and inspected traps, finding three mice. Two were located in the kitchen/dish area and one was located under the beverage station.

Observed gaps along the bottom of front doors and side door partially ajar leading to the outside at the time of inspection. Observed employee food (reusable water bottle and a partially consumed container of cut watermelon) stored on dry goods storage shelf above food items such as spices, and lemon juice. Observed no drain cover in the floor drains in the kitchen.

Observed three-compartment sink with plumbing lines in the floor drains below the flood line with no air gap allowing for the possibility backflow of sewage into the three-compartment sink.

Observed significant debris such as food items, towels, scrubbers, Brillo pads and grease behind cooking equipment such as stove and grill in the kitchen with rodent droppings present.

Observed numerous gaps/holes in walls and ceiling throughout the facility in areas such as around electrical tubing by hood system into the ceiling, around plumbing system near the dish area into ceiling and the wall, gaps in soda BIB storage cabinets, multiple holes into the wall at tubing through the wall and at the corner wall junctures at the three-compartment sink. Observed ventilation system with debris build up on it and the surrounding area.

Person in charge was issued a notice of closure and suspension and has five business days (Aug. 10) to clean evidence of rodent infestation and to ensure that no rodents are present, and to remedy the infestation such as by sealing any entry points, cleaning the facility to remove harborage, and performing pest control services.
