Person in charge unable to provide records for food handler certifications and demonstrating insufficient knowledge of foodborne disease prevention. Employee personal belongings improperly stored; facility lacked lockers or suitable storage. Nonfood contact surfaces covered in substantial debris, food waste and grease. Vomit observed in the men’s restroom toilet. Debris buildup on ventilation system. Debris buildup on flooring and walls. Tile flooring in the kitchen and warewashing area in poor repair. Missing electrical plates present in walls and ceilings. Doors leading outside in the kitchen propped open. Leaking faucet and drain pipeage at three-compartment sink. Air gaps at various fixtures. Soiled warewashing machine. Equipment and utensils cleaned without adequate prewash cycle. Broken hot water pressure gauge. Debris buildup and standing water on food contact surfaces. Facility lacked testing kit for sanitizing solution concentration and readily accessible food temperature measuring devices for cold-holding or hot-holding units. No maintained temperature logs for time/temperature control for safety food items. Observed improper thawing of frozen deli meats.
