Unlabeled working containers of toxic materials were observed. A food employee did not wash hands when required. An absorbent nonfood contact surface was observed. No quaternary ammonium testing kit was observed. Chlorine test strips were expired. Calibration logs for temperature devices were not available. Temperature logs for TCS foods were not available. Misuse of wiping cloths was observed. Soil residue was on the ice machine deflector. Ready-to-eat food had no date markings.

Juice It Up!, 9311 Coors NW (May 1)
Food removed from original packaging lacked proper labeling. Interior air ducts had debris buildup. Pests were in the facility with no control methods. Heavy rodent droppings were seen. An absorbent nonfood contact surface was observed; it was made of metal and rubber and held with duct tape. Nonfood contact surfaces had soil residue buildup; residue was in reach-in coolers, the make line and dry storage shelves. Food items in the cold holding unit measured above 41 degrees. Quaternary ammonium solution failed to meet sanitization parameters. Equipment food-contact surfaces had soil residue buildup; this buildup was on the ice machine, soda guns and utensils. Ready-to-eat TCS food lacked date markings.

Sonic Drive-In, 11715 Central NE (May 2)
Working containers of toxic materials were not properly labeled. No handwashing cleanser was available at a handwashing sink near the ice machine. No hand drying provisions were available at a handwashing sink. The hood ventilation system was not adequately collecting grease and condensation in the fryer area; grease and condensation dripped onto equipment surfaces. Soil residue had built up on nonfood contact surfaces; debris was observed in floor drains and on floors under equipment. An inadequate air gap was observed. Food was not protected from contamination in the beverage preparation area; uncovered containers of cherries, strawberries, lime and lemon wedges were seen. No toilet tissue was available in the men’s restroom. Ready-to-eat TCS food in the walk-in cooler had inappropriate date markings; frozen green chile had a nonapplicable date mark. Ready-to-eat TCS food in the slim beverage dispenser exceeded the seven-day limit; lemonade had exceeded its discard date.

Sunnyside Up, 6909 Menaul NE (May 1)
Quick oats removed from original packaging was not labeled. Working containers of toxic materials were not labeled. Employee was wearing jewelry while preparing food. Employee didn’t have proper hair restraints. An inadequate air gap was seen at kitchen floor drains. An employee was touching ready-to-eat food with bare hands. No testing kit or an expired kit for sanitizer concentration was available.
Result: Unsatisfactory; re-inspection required
Starbucks, 9371 Coors NW (May 2)
Food employees didn’t wash hands when required. Handwashing sink didn’t have a handwashing sign. Nonfood contact surfaces had soil residues; inside the counter service area. A food employee was without proper hair restraints. A food employee had unkempt fingernails, with long fake nails. Buildup and debris throughout facility. Inadequate air gaps at floor drains. Person in charge was unable to demonstrate knowledge of foodborne disease prevention. Testing kits for sanitizer were expired. A three-compartment sink was used without a rinse step. Chlorine sanitizer solution failed to meet parameters. Ready-to-eat TCS food lacked date markings, which included alternative milks.

The Craftroom, 2809 Broadbent NE (May 1)
The facility did not provide a designated area for employees to eat, drink, or use tobacco products. Front staff had animal hair on their shirt. Buildup and debris were observed on the ceiling vent covers throughout the kitchen area. Walls in the kitchen back area were in disrepair with holes. The PIC was unable to provide records of training or certification of food employees. Food temperature measuring devices were not provided or readily accessible. No testing kit or other device to measure the concentration of chemical sanitizing solution. Chemical solution utilized to sanitize food contact surfaces failing to meet parameters.

Which Wich, 8110 Louisiana NE (April 28)
Inadequate lighting was observed in the ware washing area. Dead cockroaches were not removed from traps at a sufficient frequency. Nonfood contact surfaces had a buildup of soil residues in the ice cream cooler and soda machine. Unnecessary or non-functional items with grease and soil debris were present in the kitchen and storage areas. Food employees were not wearing proper hair restraints. Floors in the kitchen and storage areas had buildup and debris, especially under equipment. The person in charge was unable to provide records of training or certification of food employees. Food items in cold holding units were measuring above 41 degrees, including tuna salad, roast beef, red onions, banana peppers and sliced tomatoes. The quaternary ammonium solution used to sanitize food contact surfaces failed to meet parameters. Equipment food-contact surfaces had a buildup of soil residues on the ice machine and can opener.

Yummy Noodle House, 6001 Winter Haven NW (April 28)
Poisonous or toxic materials like WD-40 and floor cleaner were stored above soup broth and fortune cookies, posing a potential contamination risk. Working containers of poisonous or toxic materials, such as spray bottles and wiping cloth containers for chlorine and quat sanitizer, were not properly labeled. Interior intake and exhaust air ducts had debris buildup in the kitchen directly above the food prep table. The rear door to the outside by the kitchen was propped open while not in use. A nonfood contact surface made of wood for clean dish storage was frayed with noticeable shavings, making it not smooth. A food employee in the kitchen was observed without proper hair restraints while handling food items. Buildup was observed on the boba bar wall. A soiled mop not in use was stored directly on the floor. Food items like fortune cookies, tea bags and drink ingredients were stored less than six inches off the floor in the rear hallway/boba drink bar area. Food temperature measuring devices and ambient thermometers for cold holding units were not provided or readily accessible. The food establishment did not have temperature logs available for all cold held and prepared TCS food items. Multiple food items were left out on the prep table holding at room temperature without proper temperature control, including ramen, vegetable and bok choi. The chlorine solution used to sanitize food contact surfaces in the wiping cloth bucket and three-compartment sink failed to meet parameters. Ready-to-eat time food, including chopped vegetables, prepared sauces and cooked meats, had no date marking indicators.
