Standing puddles of water in the kitchen and adjacent room were observed, indicating a contamination risk because they were unable to properly drain. Buildup and debris were observed on the floors and walls in the kitchen area. The kitchen area was observed in disrepair, specifically noting a hole in the middle with a broken pipe that was not able to drain water.
