Facility had evidence of ongoing rodent infestation. Quatemary ammonium compound solution used to sanitize food contact surfaces failed to meet parameters to achieve sanitation. Observed dead roaches and pieces of dead roaches in the electrical panels, on the ground in dry storage areas and employee break room. Gaps in exterior doors leading to outside. Exterior walls around the roof exposed gaps. Significant rodent droppings throughout the facility. Food handling employees with bracelets, watches and rings. Food handling employee with long fake nails working with food. Food handling employees with inadequate hair restraints present. Sanitizer bucket with wet wiping clothes stored on ground. Numerous employees had drinks stored in areas such as prep tables and dish racks. Boxes of single-use items stored on ground. Equipment in poor repair that could not be properly cleaned and sanitized. Single-use items stored in the dry storage/employee area with evidence of roaches and mice present. Food and grease debris on equipment. Heavy food debris throughout the entire facility. Plumbing leak from handwashing station in the men’s restroom with butter tub catching the dripping liquid. Receptacles throughout the kitchen with debris buildup. Equipment not in use partially disassembled. Food handling employees washed hands in incorrect manner.

Large pastry boxes stored at room temperature. Food thermometers were missing. No probe thermometer available. Food not labeled. Used wiping cloth stored on prep table. Improper employee coffee cup stored on prep table. Food and non-food contact surfaces improperly designed. Soiled cardboard on shelving next to sink. Improper ware washing and sanitation. No chlorine test strips available. Plumbing system not properly installed. Employee toilet room door is not self-closing. Hand-washing sinks not properly stocked.

Person in charge was not knowledgeable on food safety. Raw eggs stored above fresh produce inside reach of refrigerator. Raw ground beef stored on prep table next to food items. Multiple food items stored inside refrigerator that were not part of cooling process that were uncovered and unprotected. Chlorine sanitizer bucket tested at zero parts per million of chlorine. Red chile sauce stored at 119 degrees Fahrenheit and green chile sauce at 123 degrees Fahrenheit in hot holding at time of inspection. All food items must be reheated to 165 degrees Fahrenheit before placing in hot holding unit. Multiple food items on prep table at room temperature. Raw meet stored to thaw at room temperature. Food thermometers missing. Food packaged with improper labels. Employees preparing food with bare hands with nail polish. Used wiping cloths stored on prep table. Employee water bottle stored next to food items. Storage cabinets with clutter and debris buildup. Food debris under kitchen equipment. Hood vent filters had debris buildup. Observed employee switching job duties without washing hands. Employee grabbed eggs, potatoes and cooked meat with bare hands. No paper towels at hand-washing station.

Person in charge failed to demonstrate basic food-safety knowledge. Package of strawberries with visible mold-like substance stored in cooler. Refrigerated prep tables missing protective covers. Buildup on ice machine. Both soda machines had black soil debris buildup. Food debris inside multiple refrigerators. Food debris buildup on deli-slicing machine. Oven had food and grease buildup. Improper cold-holding temperatures for lettuce, chopped tomatoes, sliced deli meat and chopped olives. Observed items in coolers missing date labels. Multiple food items and condiments being stored in bulk containers missing labeling information. Pest control devices mounted over knife magnet and food prep table. Gap between main doors allowing sunlight into facility. Substantial frost debris inside freezer. Opened box of cookies stored in freezer. Two employees wearing wrist jewelry such as bracelets. Wiping clothes improperly stored. Soda machine at drive through window missing outer protective paneling. Hot holding table for cinnamon rolls to be in disrepair and not operational. Fans within reach of coolers had dust debris buildup. Rust and food debris in refrigerators. Missing drain cover on floor drain. Hand washing sinks had substantial lime scale and calcium in sink. Food debris throughout facility. Multiple missing or damaged electrical outlet covers. Floor drains with excessive soil and food debris buildup. Multiple missing or damaged tiles throughout the kitchen. Employee belongings such as personal coffee cup, beverage bottles and a beanie stored above food prep table. Food handling employee using phone in the kitchen and then proceed to handle food without washing hands. No signs in bathrooms instructing employees to wash hands before returning to work.
