Instructed person in charge to create a food safety culture. Facility will be required to take a food safety training course. Equipment had build up soil residues. Inadequate consumer advisory warning for the consumption of raw, undercook and otherwise treated animal products. Ambient air and water measuring decides failed to meet accuracy requirements. Wiping cloths improperly stored. Employee beverages consumed in undesignated area. Cutting surfaces with deep scratches which limit the ability to be effectively cleaned and sanitized. Non-functional mechanical warewashing machine. Soda dispensing lines, plumbing lines and floor drains not maintained in good repair. Sewage drainage from multiple outlets at the service area not draining. Instructed person in charge to contact repair personnel for repairs. Buildup of debris on floors and walls throughout the kitchen area. Hood ventilation system collecting grease and condensation. Facility operating in unsanitary conditions. Person in charge was issued a notice of closure and has five business days to send proof of corrective measures.
