Caveman Burgers & Tacos, 6205 Central NW

Hot dogs were found not hot held in a steam table above 135 degrees. Ready-to-eat, time/temperature control for safety foods like bottled salsas and cream lacked necessary date-marking indicators. Raw hamburger patties and chicken combined with ready-to-eat food were not properly date-marked. The food establishment did not have a certified food protection manager on staff. Nonfood contact surfaces, including the steam table interior and Coke dispenser, were observed with excessive soil and syrup buildup. Temperature logs for TCS foods were not available. The food establishment does not have calibration logs for temperature-measuring devices.

Guava Tree Cafe, 118 Richmond NE

The water heater was found to be in disrepair by leaking and not heating water, which meant the plumbing system was not maintained in good repair. An inadequate air gap was observed under the three-compartment sink. The food establishment did not have a certified food protection manager on staff. Food employees did not possess a food handler card or had not been trained in an approved food safety course. An uncovered container of ice was observed at the front checkout area, indicating food was not protected from contamination during storage. The facility lacked lockers or other suitable facilities for the storage of employees’ clothing or personal belongings. No food thermometer was present to check food temperatures. The facility was operating with an expired permit. The facility had insufficient hot water at the handwashing sink and the three-compartment sink in the kitchen area. Buildup and debris were observed on the fan above the cooking equipment. Nonfood contact surfaces on the exterior of cooking equipment in the kitchen area had debris buildup. The food establishment did not have temperature logs available for TCS food items. The food establishment did not have calibration logs for temperature-measuring devices.

Hong Kong Buffet, 10100 Coors NW

Two living and ten dead cockroaches, rodents, and flies were observed in the facility, requiring professional control methods. The mechanical warewashing machine lacked an accurate thermometer to measure water temperature in the wash and rinse tanks. Plumbing piping for the three-compartment sink and sanitizer dispenser was in disrepair and leaking. The backdoor screen door to the outside was not tight-fitting, allowing light and a sizeable gap due to improper gasket placement. The rear parking lot trash enclosure was unclean, with a heavy amount of garbage and liquid waste buildup on the ground. Nonfunctional items, including ovens, a fryer and miscellaneous equipment, were stored in the outdoor receiving area. An ambient air and water temperature measuring device in the standing reach-in cooler failed to meet accuracy requirements/was not in calibration. Nonfood contact surfaces, such as areas under wok stations, storage shelving, kitchen equipment exteriors and handles, were soiled with food debris, grease and soil accumulation. Floor tile grout throughout the kitchen, particularly in the dish pit and rinse areas, was completely eroded, allowing water to pool and become a harborage area for pests/bacteria. Walls, ceiling tiles and floor tiles were in disrepair, showing holes and gaps that compromised the physical facilities and a flaking ceiling tile was observed above a storage rack.

Jimmy John’s, 401 Eubank SE

The quaternary ammonium compound sanitizing solution in the wiping cloth bucket was empty and dry, failing to meet adequate sanitization parameters. Food contact surfaces on the food dicer and ice machine had a buildup of soil residue. Working containers, specifically a spray bottle containing water, were not properly labeled with the common name of the material. The person in charge was unable to demonstrate knowledge of foodborne disease prevention and other food code requirements due to multiple priority violations. An inadequate air gap was observed at the three-compartment sink. The food establishment did not have a certified food protection manager on staff. Food employees were not in possession of a food handler card or had not been trained in an approved food safety course. TCS foods in the walk-in cooler, including turkey, salami, roast beef, pepperoni, ham, homestyle potato salad, pesto bowtie pasta salad, and various salad dressings, were cold holding above 41 degrees. The facility had insufficient cold holding equipment capacity, specifically the walk-in cooler, to properly maintain TCS food cold. The testing kit used to measure the concentration of the chemical sanitizing solution was out of date. The handwashing sink at the food line prep station was observed with a buildup of soil residue, indicating use for purposes other than handwashing. The walk-in cooler had an inadequate amount of lighting, requiring the inspector to use a flashlight. A food employee was observed actively preparing open food without proper hair restraints in the kitchen area. A soiled mop, not in use, was stored improperly in a mop bucket. Buildup and debris were observed underneath the soda machine. The food establishment did not have temperature logs available for TCS foods. The food establishment did not have calibration logs for temperature-measuring devices.

Szechwan Chinese Restaurant, 1605 Juan Tabo NE

An inadequate air gap was observed at the cooler drain pipe in the floor drain near the soda syrup box storage shelf. The food establishment did not have a certified food protection manager on staff. The hood ventilation system was inadequately collecting grease and condensation. Doors to the outside were not tight-fitting, allowing light to be seen at the edges, which compromises protection against insects and rodents. Nonfood contact surfaces, including the kitchen area (grill area, ceiling, walls, and floors), had an extreme buildup of soil residues.

Thai House, 2000 Carlisle NE

An inadequate air gap was observed under the three-compartment sink. The food establishment did not have a certified food protection manager on staff. Buildup and debris were observed on the floors, ceiling and ceiling vent cover throughout the facility.
