Arriba Shihuahua, 417 Tramway NE (April 10)

No approved method of hand drying was observed at handwashing sinks in food preparation and warewashing areas. Clean equipment and multi-use utensils were observed stored in a position that didn’t allow self-draining or air drying. Utensils were stored incorrectly during a pause in use, not on a clean and sanitized portion of the equipment, with an ice scoop handle not extended in the ice machine. Clean single-service and single-use articles were observed to be stored improperly on the floor in the dry storage area. Food on display at the salsa cold bar was observed not protected from contamination with no food guards or coverings. The food establishment did not have a certified food protection manager on staff. Food employees did not have a food handler card. Chlorine sanitizing solution in the three-compartment sink was observed failing to meet adequate sanitization parameters. Ready-to-eat time and temperature-controlled food was observed with no date marking indicators in reach-in coolers. A food employee was observed without proper hair restraints in the kitchen area during food preparation.

Eat First Chinese Restaurant, 3250 Coors NW (April 10)

The facility did not provide lockers or suitable facilities for employees’ personal belongings. Handwashing sinks in the restroom were observed without handwashing signs. Equipment was not maintained in good repair, including torn and cracked plastic food storage containers. Pests were observed in the facility with no control methods in place, specifically rodent droppings in the dry storage room. Nonfood contact surfaces were observed with a buildup of soil residues, including reach-in coolers and wok station areas. An employee was observed wearing unapproved jewelry while serving rice. A food employee was observed without proper hair restraints while cooking in a wok. An inadequate air gap was observed at the kitchen handwashing station. Non-ready-to-eat raw animal food was observed stored with ready-to-eat food without protection. Food was not protected from contamination during storage in various locations. The person in charge was not able to demonstrate knowledge of foodborne disease prevention and hot holding temperatures. Food employees did not have a food handler card or have approved food safety training. Equipment food-contact surfaces were observed with a buildup of soil residues, including the ice maker and fryer strainer basket. Ready-to-eat time and temperature-controlled for safety food was observed with no date marking indicators in coolers and on make tables.

Scalo (kitchen), 3500 Central SE (April 10)

The facility did not provide lockers or suitable facilities for employee personal belongings, with personal items stored on prep tables. Poisonous or toxic materials were observed stored in a manner with the potential to contaminate utensils. No approved method of hand drying was observed at the main kitchen line handwashing sink. Heating and air conditioning system vents were not easily cleanable, with substantial debris buildup. An inadequate amount of lighting was observed at the main kitchen line hood ventilation system. A food employee was observed with an improper drink at the prep table. An employee was observed wearing unapproved jewelry while preparing food. Buildup and debris were observed underneath kitchen equipment. The hand-washing sink on the main kitchen line was observed in disrepair. Food was stored improperly, where it was exposed to contamination in refrigerators and freezers. Food employees did not have a food handler card or have approved food safety training. No chlorine testing kit was observed. The food establishment was observed to not have calibration logs for a probe thermometer. Food items in a cold holding unit at the pasta station were observed measuring above 41 degrees. The equipment food-contact surface of the ice machine was observed with debris buildup.

Smith’s, 111 Coors NW (April 11)

Cleaned equipment was observed stored improperly, less than six inches above the floor. Pests were observed in the facility with rodent droppings in storage areas and on display equipment. Nonfood contact surfaces were observed with a buildup of soil residues. Walls and floors constructed in front of the deli were observed to be uneven and not easily cleanable. The deli hand-wash sink was observed in disrepair, with a drain leaking onto the floor. A hand-wash sink faucet was observed leaking. An inadequate air gap was observed at the deli back room three-compartment sink discharge piping. Equipment food-contact surfaces of deli cases were observed with debris and soil buildup. Multiple dried rodent droppings were observed in the shelving and the back room. Holes or other gaps were observed in the structure of the facility. Excessive buildup and debris were observed in the drains of the backroom southwest cooler. The facility did not provide lockers or suitable facilities for employee personal belongings. Working containers were not properly labeled or identified. Interior intake and exhaust air ducts were observed with debris buildup. Cleaned single-service and single-use articles were stored improperly, less than six inches above the floor. Pests were observed in the facility with substantial rodent droppings in various bakery areas. Employees were observed improperly cleaning up rodent droppings. Nonfood contact surfaces were observed with a buildup of soil residues on food equipment. Unnecessary items were observed on the premises. Flooring, coving and walls were observed in disrepair with missing tiles and holes. An inadequate air gap was observed at the drain south side of the walk-in freezer. Food was stored improperly, less than six inches off the floor. Equipment food-contact surfaces of the bakery display case and pans were observed with substantial food and soil debris buildup. A mop was observed stored improperly.
