Doors to the outside were not tight fitting; light can be seen at the edges. Rodent droppings in multiple areas throughout the facility, including the back of the facility and along the back side of produce cold-holding units in the display area. Some items on the premises of the food establishment are nonfunctional. Food employee with an improper drink. Employee eating while packaging food items in an undesignated area. Build up and debris on the floor drains, and shelving in the prep-packaging area. Holes in walls in the back of the facility where shipping is received. Piping from water filter and drain from refrigeration units not maintained. Pipes in the back area where receiving occurs are leaking and causing standing water. Food employee touching ready-to-eat food with bare hands.

Facility has inadequate procedures to effectively control pests. Facility was treated for two days, though 10 live roaches were spotted on Feb. 6 in the facility storage food areas and ceiling kitchen walls, lighting, floors and entryway to the kitchen. Floor and wall junctures in the storage area south wall and south wall kitchen missing the required base coving.

Missing hood-vent filter on south wall above fryer flat-top griddle. Plastic food container storing pepper paste was stored at ambient temperature. Facility in operation had no hand sanitizer. Chemical sanitizer concentration exceeded the maximum level permitted. Two Ziploc bags storing raw chicken and raw beef had no date markings. Facility did not provide lockers or other suitable facilities for storage of employees' clothing or personal belongings. Food packaging was not in good condition and failed to protect the integrity of the contents of the food from adulteration or potential contaminants. Food that is not readily and unmistakably recognized removed from original packaging without proper labeling or identification.
